On the above date and time, Licensing Program Analyst\342\200\231s (LPA) Kelly Gerth and Sumayya Habeebulla arrived unannounced at the facility and met with Facility Owner Deseree Armstead to conduct a plan of correction visit for deficiencies: CCR 101216(f) and CCR 101227(a)(17), Issued on: November 21, 2024 and plan of correction due date of November 28, 2024. During the tour of the facility with Facility Owner Deseree Armstead, LPA\342\200\231s observed: As of the date and time of this visit, the Plan of Correction for CCR 101216(f) has been met as evidenced by: During the visit, LPA verified that the appropriate certification to meet this regulation was held by staff. Licensee also provided an LIC 855 Declaration statement of understanding of the regulation. As of the date and time of this visit, the Plan of Correction for CCR 101227(a)(17) has been met as evidenced by: The area referenced in the deficiency is off limits and not used in preparation of meals or snacks. LPA observed the snacks to be served to children in the morning and afternoon, are stored in the upper left cabinet, in sealed containers, and stored away from any cleaners/solutions. Additionally, the owner stated that the center has an updated LIC 999A Facility Sketch (Indoor and outdoor) and Parent Handbook and will be providing a copy to CCLD before March 24, 2025. An exit interview was conducted with Facility Owner Deseree Armstead and appeal Rights were discussed and provided along with a copy of this report. A Notice of Site visit was given, Facility Owner Deseree Armstead was reminded the notice must remain posted for 30 days.
