Inspector’s narrative

What the inspector wrote

The parent was not aware of the severity of the injury until they saw the child when they picked them up at the end of the day. Based upon information gathered, the preponderance of evidence standard has been met. There is enough supporting information to prove that the facility staff did not make prompt arrangements to seek medical treatment, therefore the above allegation is substantiated. An exit interview was conducted with Ms. Choi. Notice of Site Visit was provided and is to remain posted for 30 days. See 9099D for cited deficiency. Per Chapter 3, Division 12, Title 22 of the California Code of Regulations, upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. A copy of the Fact Sheet - AB 633 Child Care Parent Notification Requirements and a copy of LIC 9224 was given to licensee.
