What the inspector wrote

On 1/28/25 at 8:40 a.m, LPAs Renita Rodriguez and Hanna Lucas made an unannounced visit to follow up on self reported incident, received on 1/13/25. LPAs met with Director Vanessa Milroy and explained purpose of the visit. LPAs conducted interviews with Director and Staff and obtained evidence. LPAs observed appropriate ratios with 19 infants and 9 staff Facility self reported on 1/13/25 an incident regarding an infant pushing open a door in the classroom which leads to an outdoors area playground which is properly fenced. The first time the infant pushed open the door the alarm sounded and staff redirected child back inside the classroom. The second time the infant left the room, the alarm had not alerted staff due to not being set properly by staff after the infant had attempted to open the door the first time. The infant was seen walking outside from a classroom window by staff S2. The facility investigated the incident and the decision was for staff S1 to be released of their duties from the facility. Director stated the decision was based on internal investigation by the facility with staff involved. LPAs Renita Rodriguez and Hanna Lucas informed Director Vanessa Milroy that this report dated 1/28/25 Type A citation, which shall be posted for 30 consecutive days as there is an immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA s Renita Rodriguez and Hanna Lucas i nformed the Director Vanessa Milroy to provide a copy of this licensing report dated 1/28/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to parents/guardians of any newly enrolled child for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the Director Vanessa Milroy. A notice of site visit was given and must remain posted for 30 days.
