What the inspector wrote

On 06/16/2025 at 12:15PM,Licensing Program Analyst(LPA), Hanna Lucas made an unannounced visit to follow up on a self reported incident, received on 06/13/2025. LPA was greeted by the Director, Vanessa Milroy, and explained the purpose of the visit. LPA toured the infant classrooms(1&2), and observed 28 infants, with 4 teachers, and 2 assistant teachers. Facility is within ratio and capacity. Facility self reported an incident that occurred on 06/13/2025, regarding an infant(C1) who was found by staff member(S3), in a closet between Classroom 1 and 2, without any supervision. At the time of the incident, there were 16 children with 4 staff members present in Classroom 2. Based on interviews conducted with the Director, staff members (S1-S3), and documents related to the incident, it is unknown how the child got into the closet, but the child was found within approximately 5 minutes, and without any injury. The Director is investigating the incident, and is awaiting internal management approval for corrective action towards staff members involved. The Director is also reaching out to Technical Support Assistance, a resource provided by the Department to help facilities understand and algin within regulations. LPA confirmed that C1 was left without supervision and therefore violates Title 22 of the CA Code of Regulations. A Type B deficiency is cited on the LIC809-D. Exit interview conducted and report was reviewed with the Director, Vanessa Milroy. Appeal rights were provided and discussed. A NOTICE OF SITE VISIT WAS GIVEN AND MUST BE POSTED FOR 30 DAYS.
