What the inspector wrote

On 07/14/2025 at 11:00AM, Licensing Program Analyst (LPA) Hanna Lucas made an unannounced case management visit to follow up on a self reported incident that occurred on 07/07/2025 wherein a child (C1) chocked on a Band-Aid. There were 29 infants with 10 teachers present during today\342\200\231s visit. Facility was within ratio and capacity. LPA toured Classroom 1, where the incident took place, and observed the First Aid supplies, that were kept in a cabinet above the sink, inaccessible to children. LPA interviewed the staff involved (S1 thru S3), and gathered documents pertaining to the incident. At the time of incident, there were 3 teachers and 10 infants. (S1-S3) confirmed that the parent of (C1) had told the facility that the child has received some vaccines, but did not specify that the child had Band-Aids on. (S2) noticed there were two Band-Aids on (C1) during morning diaper changes, and removed one Band-Aid, that was loose and located on the child\342\200\231s toe. (S2) did not remove the additional Band-Aid, as it was located on the upper thigh, covered by the child\342\200\231s clothing, and was skin tight. Later on in the afternoon, (S3) observed (C1) chewing on a Band-Aid, and it was taken from the child and thrown away. A couple minutes later, (S2) and (S3) observed (C1) in distress, and took proper procedures to clear the child\342\200\231s air way. (S2) confirmed that she observed part of A Band-Aid stuck in the child\342\200\231s mouth, causing the child to choke. Staff notified the Director, Vanessa Milroy, who contacted emergency services and the parents of the child. (C1) was cleared by the paramedics, and returned to school the following day. Proper steps were taken, ratios were met, and supervision was in place. It does not appear that the actions or inactions of the facility or facility staff contributed to the incident. No deficiencies cited today.
