What the inspector wrote

On 5/14/25 at 12:05 p.m, Licensing Program Analyst LPA Renita Rodriguez made an unannounced visit to follow up on self reported incident, received on 4/14/25. LPA met with Assistant Principal Melissa Thomas and explained purpose of the visit. LPA conducted interviews and obtained children's roster. Ratios observed during nap time: 132 children and 8 staff. Facility self reported on 4/14/25 an incident involving a child and staff member which occurred on 4/11/25. A visitor at the facility is said to have witnessed a child in care and their personal rights being violated. Staff S2 also witnessed the incident involving S1 and a child. S2 witnessed and heard S1 speaking in a loud tone to a child C1. The facility investigated the situation and the decision was for staff S1 to be provided with a written corrective action. See LIC 809D for deficiency cited. Exit interview conducted with Assistant Principal Melissa Thomas. Notice of Site Visit was given and must remain posted for 30 days.
