On April 21, 2026, at 9:15 AM, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced Case Management inspection to follow up on a self-reported incident. LPA met with Facility Representative Teri Potts. LPA disclosed the purpose of the inspection and toured the indoor and outdoor of the facility. Days and hours of operation are Monday through Friday from 8:30 AM to 11:30 AM. There were 14 children present with three (3) staff members. At 10:45 AM, Facility Representative Charmaine Lawson arrived to the facility. Facility Representative Charmaine Lawson reported that on March 4, 2026 at approximately 11:25 AM, there was an alleged personal rights violation reported involving Child #1 (C1). LPA provided LIC 811 Confidential Names List to Facility Representative Charmaine Lawson. Interviews were conducted with the facility representatives and staff members. C1 declined to be interviewed. LPA reviewed staff files and obtained copies of facility roster. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Facility Representative Charmaine Lawson.
