On June 3, 2026, at 11:46 AM, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced Case Management inspection to follow up on a self reported incident. LPA met with Facility Representative Charmaine Lawson and disclosed the purpose of the inspection. Days and hours of operation are Monday through Friday from 8:30 AM to 11:30 AM. There were 11 children present with three (3) staff members. Facility Representative Charmaine Lawson reported that on March 4, 2026, at approximately 11:25 AM, there was an alleged personal rights violation reported involving Child #1 (C1). LPA provided LIC 811 Confidential Names List to Facility Representative Charmaine Lawson. Interviews were conducted with facility representatives, staff members and the authorized representative for C1. C1 declined to be interviewed. The witness that allegedly observed the incident was not available for interview. LPA reviewed staff files and obtained copies of facility roster. Due to insufficient evidence obtained, LPA is unable to determine if a personal rights violation occurred. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Facility Representative Charmaine Lawson.
