What the inspector wrote

On 3/10/23 at 11:30 AM Licensing Program Analyst (LPA) Adrian Mangina made an unannounced complaint visit for the complaint received on 2/9/23 for the purpose of continuing the investigation of the above reference allegation. During this visit LPA brienfly toured facility. While there, LPA observed that the sharks and Dolphins classrooms were supervised by an aid who supervised by an unqualified teacher. During the visit LPA, continued staff file reviews from previous visit, and interviewed staff. The allegation above is valid because the preponderance of the evidence has been met, therefore, the above allegation is found to be SUBSTANTIATED. The deficiency is being cited on the attached LIC 9099D. Exit interview conducted and report was reviewed with the facility representative Melinda Carvalho. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Based on observation and record review he licensee did not comply with the section cited above as eight infants were supervised by an unqualified teacher and an aid which poses a potential health, safety or personal rights risk to persons in care.
