On 4/1/25 at 1:05 p.m., LPA Renita Rodriguez made an unannounced visit to follow up on self reported child incident report, received on 3/18/25. Upon entry, LPA met with Director Melinda Carvalho and explained purpose of the visit. Ratios observed 98 and 16 staff. LPA conducted interviews, made a confidential names list, observed the playground, and received a copy of the children’s roster. Staff S1 observed child C1 running on the playground, tripped backwards, fell and hit the back of the head. 9-1-1 called and parents were contacted. Child was taken home by parent. Child received medical attention by pediatrician on the same day as incident. Child returned to the facility the next day without medical directives. No deficiency cited. Exit interview conducted and report was reviewed with the Assistant Director Brtitney Mccray. A notice of site visit was given and must remain posted for 30 days.
