What the inspector wrote

During interview, Director stated that this incident was not reported timely to Licensing because she initially believed it was not a reportable incident. Director stated she later became aware that S1 had not maintained visual supervision of C1 and reported the unusual incident to Licensing via an Unusual Incident Report (LIC624) on 01/31/2025. Based on information obtained in interviews, the preponderance of evidence standard has been met and the above allegations are deemed Substantiated. California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 9099-D. Upon Receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. Exit interview conducted and copy of report and appeal rights were provided to the Director. A notice of site visit was given and must remain posted for 30 days.

Supervision shall include visual observation.This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above as S1 did not maintain visual supervision as Child 1 (C1) ran out of the building and down the stairs, which posed an iimmediate health, safety, and personal rights risk to persons in care.
