What the inspector wrote

On 10/6/2021 @ 10:20AM, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced case management inspection. This is in reference to facility's self-reported incident that occurred on 8/19/2021 wherein a 3-year old child was left unsupervised in the play yard for approximately 3-5 minutes. This child was found in the play yard by parent during pick up. LPA observed 15 preschool children present today with staff Dori Holm, Tyler Erwin and Kelli Morrissey. A tour of the facility was conducted with Ms. SalcIdo. She stated that a substitute staff was supervising during outdoor play when one child had to use the bathroom. Staff failed to conduct the name to face check before transitioning from the play yard to the bathroom. Type A violation if not corrected, will have a direct and immediate risk to the health, safety, or personal rights of children in care. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. An exit interview was conducted with Ms. Salcedo. Appeal rights were discussed and a written copy was provided today. A notice of site visit was observed posted, this notice shall remain posted for 30 days.

Supervision shall include visual observation.This requirement was not met as evidenced by: Based on the documentation provided to the department and information provided by Ms. Salcedo. Licensee reported that a substitute staff failed to provide visual supervision when she transitioned her group to the bathroom, leaving a 3-year old child alone in the play area.
