What the inspector wrote

On December 3, 2025, 9:50AM, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced Annual Random inspection. LPA met with Facility Representative, Hope Sanchez and disclosed the purpose of the inspection. At 10:30AM, Facility Representative, Charmaine Lawson arrived to the facility. LPA toured the indoor and outdoor of the facility. T his is a half day program that operates on a traditional school year. Days and hours of operation are Monday \342\200\223 Friday, 8:30AM. \342\200\223 11:30AM. There are two adjoining classrooms in operation. The following ratios were observed: Rooms 23 & 24 - 16 children present with, one (1) teacher and two (2) paraprofessionals. Furniture and equipment are in good condition. The floors in the facility are clean and safe. Toilets and hand washing facilities are in safe and sanitary operating condition. Disinfectants, cleaning solutions, medications and other hazardous items were made inaccessible to children during time of inspection. Food preparation and storage areas are clean, free of litter and rubbish. Solid waste storage containers have tight fitting covers and are in good repair. The playground equipment is in safe condition. The surface of the outdoor activity space is maintained in a safe condition and free of hazards. Areas around high climbing equipment and slides have cushioning material to absorb falls. Facility has smoke and carbon monoxide detectors that meet statutory requirements. The last fire drill was conducted and documented on November 20, 2025. Facility Representative stated there are no bodies of water on the premises. Facility Representative stated that there are no firearms, ammunition or weapons stored on the premises. See LIC 809C Continuation... Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Facility Representative Charmaine Lawson stated that all employees working at the state preschool have received criminal record clearance through the Spring Valley La Mesa School District. Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher, supervising no more than eight (8) children in care. At least one person trained in Pediatric CPR and First Aid is present when children are at the facility. The name of the facility representative and the fully qualified teacher designated to act in the facility representative\342\200\231s absence has been reported to the Department. LPA reviewed sign in/sign out sheets and they are being maintained. Facility Representative was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children\342\200\231s files and observed files were complete with contact information for authorized representatives and or relatives or others who can assume responsibility for the child and medical assessment. Drinking water is available both indoors and outdoors. Facility Representative stated that breakfast is provided by the Sweetwater Springs Elementary School Cafeteria. Menus are posted monthly. LPA reviewed a sample of staff files and observed files to be complete with immunization records for influenza, pertussis and measles and current documentation of mandated reporter training. Facility Representative stated that all employees working at the state preschool have completed health screenings. See LIC 809C Continuation... For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 ( PIN 21-21.1-CCP .). See LIC 809C Continuation... LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP . LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ) Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A notice of site visit (LIC 9213) was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Facility Representative, Charmaine Lawson had to leave the facility and authorized Facility Representative, Hope Sanchez to sign facility report. Exit interview conducted and report was reviewed with Facility Representative, Hope Sanchez.
