What the inspector wrote

On December 3, 2025, at 11:31AM, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced Case Management inspection regarding a self reported incident. LPA met with Facility Representative Charmaine Lawson and disclosed the purpose of the inspection. LPA accompanied by Facility Representative toured the outdoor of the facility. This is a half day program that operates on a traditional school year. Days and hours of operation are Monday \342\200\223 Friday, 8:30AM \342\200\223 11:30AM. There are two adjoining classrooms in operation. The following ratios were observed: Rooms 23 & 24 - 16 children present with, one (1) teacher, two (2) paraprofessionals. Facility Representative, Charmaine Lawson reported that on November 13, 2025 at approximately 10:32AM, Child #1 (C1) was found alone and without supervision outside of the children\342\200\231s restroom between the path from the restroom to the classroom by two school district staff members. Interviews were conducted with the facility representative, staff members and C1's authorized representative. C1 was not available to be interviewed. LPA reviewed staff and children files, obtained copies of facility roster, sign in/sign out sheets and related documentation. LPA took photos of the outdoor area where C1 was found. Based on interviews conducted, C1 left the children\342\200\231s restroom unnoticed and when found was observed walking back in the direction of the classroom. See LIC 809C Continuation... Facility Representatives stated that C1 was initially in the restroom with Staff #1 (S1), Staff #2 (S2) and eight daycare children including C1. S1 and S2 stated that they were assisting other daycare children during the time that C1 left the restroom. Upon preparing to leave the restroom and conducting a head count, S1 and S2 immediately realized that C1 had left the restroom. S1 immediately radioed on the walkie talkie to the staff on campus advising them that C1 had left the restroom unsupervised. Staff members acknowledged that there is not a direct line of sight from the children's restroom to the classroom. C1 was immediately located in route to the classroom, which is between the children's restroom and the classroom. Per staff, C1 was left unsupervised for approximately less than 1 minute. LPA inspected the route taken by C1. The area outside of the classroom is fenced and all entrance gates are locked throughout the day. Based on interviews, it was determined that C1 was left alone and without staff supervision, between the path of the children\342\200\231s restroom and classroom area for approximately less than 1 minute. Facility Representative stated that a revised plan for supervising children in the bathroom has been implemented and the plan has been discussed with classroom staff. Per California Code of Regulations, (Title 22, Division 12, Chapter 1), a deficiency is being cited on the attached LIC 809D. LPA Vicky Williamson informed Facility Representative, Hope Sanchez that this report dated December 3, 2025 document(s) (one) of Type A citation Type A citation(s) which shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Vicky Williamson informed the Facility Representative, Hope Sanchez to provide a copy of this licensing report dated December 3, 2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. See LIC 809C Continuation... A notice of site visit (LIC 9213) was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Facility Representative, Charmaine Lawson had to leave the facility and authorized Facility Representative, Hope Sanchez to sign facility report. Exit interview conducted and report was reviewed with Facility Representative, Hope Sanchez.
