What the inspector wrote

On 10/8/2025, at 9:45am, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced Case Management inspection to follow up on a self - reported incident. LPA met with Facility Representative, Hortensia Murillo and disclosed the purpose of the inspection. LPA accompanied by Facility Representative toured the indoor and outdoor of the facility. This is a full day program which operates on a traditional school year schedule. Days and hours of operation are Monday \342\200\223 Friday, 8:00am \342\200\223 4:00pm. There were 66 children present, 37 of whom are ages 18 - 36 months present with 14 teachers. Facility Representative reported that on 9/24/2025 at approximately 10:00am there was an absence of supervision involving Child #1 (C1). Interviews were conducted with the facility representative and staff. C1 declined to be interviewed. LPA reviewed staff and children files, obtained copies of facility roster, and related documentation. Facility Representative will submit the sign in/sign out sheet, to LPA, no later than 10/14/2025. LPA and Facility Representative reviewed and discussed methods on how to complete the Unusual Incident/Injury Report form LIC 624. A Notice of Site Visit (LIC 9213) was given to Facility Representative, Hortensia Murillo and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and the report was reviewed with the Facility Representative, Hortensia Murillo.
