LPAs (Licensing Program Analysts) Saraliz Velando and Hanna Lucas conducted an unannounced follow up on a self-reported incident that occurred on 1/17/25 where a child fell off a bar and obtained an injury to the right elbow. The LPAs were greeted by the Director, Paula Mercer. LPAs toured the facility and there were 106 preschool children and 19 staff present today. LPAs conducted staff interviews and determined that the child had sufficient staff supervision and the classroom was in ratio at the time of the incident. LPAs also observed that the Big Playground area has enough cushion under the playground equipment and is in good condition. An exit interview was conducted with the Director Paula Mercer. The Appeal Rights with a copy of the report was provided. The notice of site visit was posted and must remain for 30 days.
