On July 9, 2025, at 10:10AM, Licensing Program Analyst (LPA) William Chancellor arrived at EES- David and Jillian Gilmour Early Education (CCC) and was granted entry to the facility by Director (DIR) Delilah Rodriguez. A second case management visit is being conducted in response to the receipt of an Unusual Incident Report (UIR) received by Community Care Licensing (CCL) on June 23, 2025. DIR self reported an incident to the duty officer involving a child (C1) being injured on the playground by temporary construction fencing and requiring medical attention. The UIR was reviewed and confidential interviews were conducted with four staff (S1-S4). Documents relevant to the incident were obtained and confirm that the facility was within ratio at the time of the incident. Three of three interviews revealed that no staff observed the incident directly, however children stated that child 1 (C1) was hit in the face with a post. Staff did confirm observing the post loose and hanging in the area, after C1 directed showed them where the incident occurred. It cannot be corroborated at this time that the pole hit C1 in the face due to falling or being pulled by a child. Observations confirm that the CCC has taken appropriate measures to reinforce the fencing posts, tape off sharp ends, stagger recess schedules and redirect children to play in alternative areas. Based on information gathered, the facility acted appropriately, and there are no deficiencies cited at this time. An exit interview was conducted, and a copy of this report was provided to Director, Delilah Rodriguez. A Notice of Site Visit was provided and must remain posted for 30 consecutive days.
