On June 1, 2023 at 1:10 pm, Licensing Program Analyst (LPA) Jessica Rubio arrived unannounced to the EES Foothill Oaks Preschool (CCC) to conduct a case management visit due to an unusual incident report received for an incident that occurred on 5/24/2023, involving child (C1) alleging C1 was hit in the face by a teacher. LPA met with Director (AD) Nicole Welty-Hazim and conducted a tour of the facility. During the visit, LPA conducted interviews with Director and the one other staff (S1) who works in the facility. LPA also followed up with Program Director (PD) Vianna Ramnath by telephone. Interviews revealed C1 attended the CCC for approximately three weeks and the day the allegation was made, C1 had a behavioral incident which S1 addressed with C1 by talking to C1 and Director called C1's parent to inform them of the incident. It was alleged later that day that an unnamed teacher hit C1 in the face. Interviews with Director and staff revealed neither of them had hit C1 nor had they ever seen one another hit C1 the day of the incident or any other day C1 attended. The interview with PD revealed, C1 was moved to another site at the parent's request and C1's parent later recanted the allegation and no longer had any concerns. LPA determined that the facility was not in violation of Title 22 Regulations. An exit interview was conducted, a copy of this report, LIC 811 (Confidential Names List) and appeal rights were reviewed with and provided to Director Nicole Welty-Hazim. A notice of site visit was also provided and must remain posted for 30 days.
