What the inspector wrote

On August 29, 2025, at 8:25 a.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Annual inspection. LPA met with Facility Representative, Lynn Leavitt and disclosed the purpose of inspection. LPA was led on a tour of the inside and outside of the facility by the Facility Representative. There were no day care children and one teacher present during the inspection due to the facility being closed for a teacher preparation day. This facility operates a full day year round program. The facility serves age newborn \342\200\223 24 months in the Room #302. Hours of operation: Monday \342\200\223 Friday 7:30 a.m. to 5:30 p.m. Disinfectants, cleaning solutions, and other hazardous items were made inaccessible to the children in care. No poisons were observed during the inspection. Furniture and equipment in the classroom was observed to be in good condition, free of sharp, loose or pointed parts. Infant changing table were within arms reach of the sink. Areas around high climbing equipment were observed to have sufficient cushioning material by the use of a carpet to absorb falls. Shade was observed in the outdoor activity space by the use of shade kite canopy. Drinking water is available both indoors and outdoors. Facility Representative stated that there are no swimming pools or other bodies of water on the premises. Facility Representative stated there are no firearms or ammunition allowed or stored on the premises. The facility has a functioning carbon monoxide detector and a first aid kit that meet statutory requirements. The last fire drill was conducted and documented on 5/13/2025. Earthquake drill was conducted and documented on 4/15/2025. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than four (4) children in care. The name of the facility representative and the fully qualified teacher designated to act in the facility representative\342\200\231s absence has been reported to the Department. LPA reviewed facility physical sign in /sign out sheets. Facility Representative was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children\342\200\231s files and observed the files were complete with contact information for authorized representative and/or relatives or others who can assume responsibility for medical assessment. Bottles containing formula contain the child\342\200\231s name and date. Infant Needs and Service Plans were reviewed and determined to be complete. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. A Notice of Site Visit (LIC 9213) was given to Facility Representative, Lynn Leavitt and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview as conducted and this report was reviewed with Facility Representative, Lynn Leavitt.
