Follow-up on corrections·September 13, 2022

On September 13, 2022 at 2:10 p.m. Licensing Program Analyst (LPA) Leilani Curtis conducted an unannounced case management inspection. Upon arrival LPA met with Director Shannon Farrell and proceeded to tour the facility. There were 30 children present with 4 staff members. Appropriate ratio/capacity was observed. Staff members have the required background clearances and are associated to the facility. On 6/27/22 and 7/6/22 LPA reviewed staff records at the facility. At the time of the inspections the director did not have a copy of a teacher\342\200\231s (S1) transcript/education record. According to the Guardian Background Check System this person was associated to the facility from 5/18/22-6/15/22. The director states that S1 last worked as a teacher on site 6/9/22. On 6/29/22 LPA received an email from the director stating that this staff member is no longer an employee and they do not have a copy of the teacher\342\200\231s transcript. See LIC809D for cited deficiency. An exit interview was conducted with Director Farrell and appeal rights (LIC 9058) were discussed. A copy of this report as well as a copy of the appeal rights were given to Ms. Farrell. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed the director post Notice of Site Visit.
