What the inspector wrote

On November 17, 2025, at 12:30 p.m., Licensing Program Analysts (LPA\342\200\231s) Cindy Meier and Raina Alexanian conducted an unannounced Case Management visit for the purpose of follow-up on an Unusual Incident Report that was submitted to the San Diego Regional Office. LPA met with Facility Representative, Site Director, Araceli Ongyan and discussed the purpose of the inspection and was led on a tour of the facility. There were eighteen (18) children present with five (5) staff members at the time of the inspection. On 11/7/2025 around 12:35 p.m., Staff 1 (S1) was transitioning (10) children from the classroom to the hall bathroom. Due to child (C1\342\200\231s) spitting behavior throughout the morning, S1 administered tape on C1\342\200\231s mouth to prevent C1 from continuing to spit. Staff (S2) observed the tape on C1\342\200\231s mouth, questioned S1 and advised the Director. According to the Director and S2, the tape was removed after approximately two (2) minutes. C1 testified to this action being conducted from S1. Director stated an Unusual Incident Report was emailed to Licensing on 11/13/25. Director stated staff received training in children\342\200\231s Personal Rights and appropriate discipline on 11/31/2025. During this investigation, LPA obtained facility records, S1\342\200\231s written account of incident via email, incident report, video footage, and interviews from Director, S2 and C1. Based on the video footage reviewed, S1\342\200\231s admission in a written statement of the incident, and interviews, it is determined that inappropriate discipline was conducted from S1, and the Personal Rights of C1 were violated. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, one (1) Type A deficiency was cited; see LIC809D. LPA Cindy Meier informed Facility Representative, Site Director, Araceli Ongyan, that this report dated 11/17/25 document(s) (1) Type A citation(s) which shall be posted for 30 consecutive days as there is an immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Cindy Meier informed the Facility Representative, Site Director, Araceli Ongyan to provide a copy of this licensing report dated 11/17/25 that documents the Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative, Site Director, Araceli Ongyan.
