Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

101216.3 (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance...This requirement was not met as evidenced by: Bssed on interviews, record review and LPA observation. On this date, a teacher was alone with 18 children, while an aide was alone with 11 children. This poses a potential risk to the health, safety or personal rights to persons in care.

101538.3 (b) In combination programs, indoor activity space provided for school-age child care center children shall be physically separated from space provided for infant care and child care center children.This requirement was not met as evidenced by: LPA observed 3 school age children in the classroom with preschool age children, which poses a potential risk to the health, safety or personal rights to persons in care.
