What the inspector wrote

It was alleged that Child 1 (C1) has had a dirty diaper several times upon pick up. 4 out of 4 staff interviewed stated that they check the children every 2 hours and change as needed. Regarding C1, staff indicated they change C1 prior to going outside after nap time. It was stated C1 typically gets picked up while they are still on the playground and that there have been times where the child had a dirty diaper, but parent declined to have the child changed prior to leaving the facility. Staff stated they have begun doing extra checks of C1 while on the playground and prior to parent picking up child. Based on record review LPA observed diaper logs and attendance records for months 9/2024 thru 2/2025 and seen that C1 is being checked/changed every 2-3 hours, if not sooner. Staff disclosed they do not document each diaper check but moving forward will document each check in their app. Regarding the allegation that staff did not safeguard a day-care child's personal belongings, it was disclosed by 4 of 4 staff members do their best to keep the sippy cups accounted for, however, C1 has had a few sippy cups go missing. It was disclosed by confidential witness that out of the 3 sippy cups that went missing, 2 cups were replaced and 1 was located over the course of a year. Staff also stated that they now have carriers for the children to place their sippy cups in to keep them accounted for. Lastly, based on interviews conducted for statement alleging child has had instances of sand all over the face and head, it was disclosed by 4 out of 4 staff members that they always dust the children off before they come back from being outside. Staff stated they do their best to get all the sand off the children but while they are active at play on the playground it can be a challenge. It was disclosed by confidential witness that C1 has been picked up while on the playground 2 times and had sand over them. Staff stated that 1 of these instances C1 and another child had just dumped sand on each other. Based on the information obtained during this investigation, it has been determined that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur. Therefore, the allegations are UNSUBSTANTIATED . An exit interview was conducted, and this report was reviewed with the Principal, Melinda Lopez , and a copy was provided. Appeal rights were discussed and provided during the exit interview.
