Licensing Program Analyst (LPA) Keely Messerschmidt arrived at the facility on a case management inspection to follow-up on an Unusual Incident Report (UIR) which occurred on September 26th, 2025 per Director. LPA met with Director Geneva Loken-Williams, and provided purpose of inspection. At the time of inspection, LPA toured the facility, took census, interviewed and reviewed documents previously submitted to the department with Director. LPA interviewed 3 staff members during visit. Based on interviews conducted 6 of 6 staff members confirmed that it was stated that Parent #1 (P1) had brought Child #1 (C1) in the classroom from the hallway where they were alone. Based on witnesses interviewed, it was stated that C1 was seen in the hallway alone and was brought into the classroom. Based on interviews conducted, it is unknown the time frame C1 was left alone in the hallway. LPA reviewed document's with Director that show C1 may have been alone in the hallway for approximately 1 to 8 minutes. A Type B citation for Lack of Supervision has been issued. An exit interview was conducted and a copy of this report was provided. Notice of site visit was provided and must remain posted for 30 days.
