On 01/15/26 at 1:00 pm, LPA (Licensing Program Analyst) Gerald Poindexter, conducted an unannounced annual inspection. LPA disclosed the purpose of the inspection and was granted entry by Lien Mai, center director, who was later joined by Christine Kutzner, preschool operations supervisor. Hours of operation are from 7:30 am to 5:30 pm, Monday through Friday. There were 22 children present with two teachers, one aide -- all observed just prior to naptime. Facility is within ratio and capacity. LPA toured the facility. The room was clean, orderly and at a comfortable temperature during this visit. Adequate ventilation and heating are available. The furniture, books, games and toys are safe, age-appropriate and in good repair. An activity schedule is posted and there is a variety of activities available throughout the day. Floors appear to be clean and safe. Hand washing and toileting areas are in safe, sanitary, and in operating condition. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Any wastewater used to clean is being discarded after use. All storage containers and trashes containing solid waste have tight fitting lids and are in good repair. Poisons, disinfectants, cleaning solutions and other items that are dangerous to children have been made inaccessible. There is no evidence of rodent or insect activity. Ms. Kutzner stated there are no firearms or other weapons on the premises. Drinking water is readily accessible.Food service is optional from the main kitchen on campus. However, most parents provide snacks and lunch. The classroom also maintains a small supply of snacks that are stored safely, separate from any chemicals or cleaning products. Medications are kept locked, inaccessible to children, and LPA observed that they were properly labeled with child name, as well as... CONTINUED ON PAGE 2 with parent permission, instructions and expiration dates. Children are evaluated upon entry and monitored throughout the day for signs of illness. Sick children are isolated within the classroom while awaiting pick up or sent to the nurse's office if unable to isolate. Outdoor play area is fully fenced with sufficient cushioning and adequate shade, separate from other programs. Portable water is used outdoors. Age-appropriate playground equipment and outdoor surfaces are in a safe condition with any equipment securely bolted to the ground. There are no bodies of water at the facility. The carbon monoxide and smoke detectors are operational. The facility has a written disaster plan in place that meets regulatory requirement and has been conducting and documenting evacuation drills every six months with most recent on 1/13/26. The facility does not transport children. Sleep equipment was reviewed and is in good repair. Parents supply bedding and cots are cleaned and disinfected every week. LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. All required licensing forms were posted. LPA reviewed a sample of children\342\200\231s, personnel, and administrative records. LPA reviewed manual sign in/out sheets. LPA reminded director to maintain all sign in/out records for at least 30 days. There are staff present with current CPR and First Aid certification. Facility is reminded the Mandated Reporter Training is to be retaken every two years and can be accessed at the following website: www.mandatedreporterca.com . This facility provides Incidental Medical Services \342\200\223 IMS. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead CONTINUED ON PAGE 3 contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Ms. Kutzner was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA also informed Ms. Kutzner of the importance of checking for recalled devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Ms. Kutzner was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights provided.
