What the inspector wrote

On 07/05/2023 at 3:00 PM, LPA Dana Stevens, conducted an unannounced complaint visit to deliver findings on the above allegation. LPA met with Director, Maria Enriquez, and informed her of the purpose of the visit. There were 25 children present with 5 staff at the time of the inspection. During the investigation LPA conducted two unannounced inspections of the facility, interviewed Director, Assistant Director, teachers, daycare children, daycare parents, and reviewed facility records and facility security video footage provided by Director. Information obtained in interviews and review of facility records and video footage from 04/24/2023, did not provide enough evidence for LPA to determine if the mark on Child 1's (C1's) neck was sustained while in care, thus the allegation Day care child sustained an unexplained mark while in care, is deemed Unsubstantiated. A finding that the complaint is Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations occurred. No deficiencies cited. The Notice of Site Visit (LIC 9213) is to be posted for thirty (30) days. An exit interview was conducted. Licensee Rights (LIC 9098 01/16) along with a copy of this report was provided to staff; their signature on this form confirms receipt of these rights.
