What the inspector wrote

On 02/03/26 at 10:00 AM, Licensing Program Analysts (LPAs) Gerald Poindexter and Jennifer Irving conducted an unannounced case management inspection to follow up on an unusual incident that was self-reported on 1/16/26. Upon arrival, LPAs met with and interviewed Director Leslie Heater. LPAs toured classrooms reviewed facility documents. LPA observed a total of 27 preschoolers in the following classrooms: \302\267 Room 1 had 12 children with 2 staff members \302\267 Room 2 had 15 children with 2 staff members Appropriate ratios and capacity were observed. LPA observed appropriate care and visual supervision during the inspection. According to Ms. Heater, on 1/16/26 at approximately 12:04 pm, staff S1, the lead teacher, began transitioning 7 children from Room 1 to the bathroom. Upon arriving at the bathroom, they only had 6 children, present \342\200\223 a fact that emerged when another teacher S2 found one child on the walkway leading to the bathroom. The child C1 had been left behind during the transition. Ms. Heater estimates the time the child was alone at about one minute, since S2 had just returned from lunch and clocked in at 12:05 pm. Another teacher, S3, took the child to the bathroom to join S1 and the other children. Ms. Heater states she was informed of the incident at 2:15 pm by S3, and shortly, thereafter, by S1. The child\342\200\231s mother was informed at approximately 2 pm by S1. Following the incident staff S1 was placed on administrative leave. Ms. Heater stated that she has reviewed classroom transitions with the staff, as well as principles of active supervision. Pursuant to Title 22 of the CA Code of Regulations, the following Type B deficiency was cited (see LIC809-D). Exit interview conducted and report was reviewed with Leslie Heater, director. Notice of Site Visit was provided and must remain posted for 30 days. Appeal Rights provided.
