On 2/6/26 at 11:19 a.m., Licensing Program Analyst (LPA), Renita Rodriguez, made an unannounced visit to evaluate the circumstances surrounding a self-reported incident that occurred on 1/23/26 wherein C1 required medical attention. Ratios observed today were 20 children with 5 staff. During today's visit, LPA conducted interviews, according to the information obtained, the ratio for classroom 1 during time of the incident was 9 children and 4 staff. Classroom 1 was engaging in outdoor activities. C1 was playing on the play ground when suddenly child was observed laying on the turf ground on their side. Staff were calling out to child and child did not respond. Staff S1 picked up child and entered into the facility. Center Supervisor Renee McBride called 911. The child became responsive during the phone call with 911. Medical attention was provided on site and parents arrived. Parents took child to pediatrician and child returned to the center on 1/28/26 with medical documentation and no limitations. LPA obtained documents. Notice of Site Visit was given and will be posted for 30 days.
