Inspector’s narrative

What the inspector wrote

On 03/23/2023 at 2:15 PM, Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced inspection to deliver complaint findings for the above allegation. LPA met with Director Natalija Worrell and discussed reason for visit. There were 22 children and 3 teachers present at the time of the inspection. It was alleged that the Facility has an infestation of rats. During the course of the investigation, two unannounced inspections were conducted. Interviews were conducted with school staff, teachers, children, parents, and pest control service providers and LPA reviewed school records. The school provided proof that monthly pest control service has been active since August 2022 and record of recent pest control inspection results was obtained. Due to conflicting information obtained during the course of the investigation, the above allegation is deemed to be UNSUBSTANTIATED meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged allegation occurred. Exit Interview conducted and copy of today's report, Notice of Site Visit and appeals rights given to the licensee. Notice of Site Visit should be posted for 30 days from today's date. This is an amended version of the original report dated 03/23/2023.
