On 08/05/24 at 10:40AM, LPA Luigi Gargaro conducted an unannounced complaint finding delivery visit to the facility regarding the above allegation. During the course of the investigation, analyst conducted interviews with the reporting party, the facility director and owner, staff members and a related day care parent. Analyst also reviewed documentation relevant to the allegation. From the evidence gathered, it could not be conclusively proved or disproved that a parent was denied access to his child's records upon request. All interviewed facility personnel involved with the incident denied that a request for records was ever made and there was insufficient corroborative evidence to either substantiate or refute the allegation. Therefore the allegation was determined to be unsubstantiated. An exit interview was conducted and the report was reviewed with facility director Analilia Lopez. A copy of this report, along with Appeal Rights (LIC9058 01/16), were provided. A notice of site visit was given and must remain posted for 30 days. LPA observed that the notice of site visit was posted during the inspection. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
