What the inspector wrote

Licensing Program Analyst\342\200\231s (LPA\342\200\231s), Keely Messerschmidt and Courtnee Peebles and Licensing Program Manager (LPM) Carlos Martinez conducted an annual inspection as part of a compliance review. This is a combination center and the other licensed program is 376700985 which were also inspected on April 6 th 2023. A tour of the inside and outside of the facility was granted and the following was observed and/or noted: \302\267 The following were posted and are updated where necessary: - License - Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148) - Parent\342\200\231s Rights Poster (PUB393) - Personal Rights (LIC613A) - Child Car Seat Law \302\267 The facility is operating with the limits as stated on the license \302\267 Ratios are being met during this inspection with a ratio of five children and three staff \302\267 Classrooms are adequately equipped with age and size appropriate furniture and equipment and are not free of hazards, during the facility tour LPA and LPM observed a whole in the wall of classroom two behind the door. A citation will be issued. In classroom 5 LPA\342\200\231s and LPM observed a wet spot on the ceiling that contained mold above the television. A citation will be issued. During the facility tour LPA observed a non operable smoke detector in classroom 2, which poses a potential health and safety risk to persons in care. A citation will be issued. In classroom five LPA observed an empty first aid kit. LPA advised S1 to restock the first aid kit with necessary equipment. (I.E.. Bandaids, scissors, tweezers..etc) \302\267 There are no weapons present at the facility as stated by Jessica Beverly \302\267 There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Drinking water is provided in the indoor activity space by filtered public water \302\267 A review of staff records on indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. S1 is not associated to the facility, which poses an immediate health and safety risk to persons in care. A citation will be issued. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. However, during the facility tour LPA observed a menu from May 2022. LPA reminded S1 that a current menu should be posted for parent review. \302\267 The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall \302\267 The Licensee was informed of their reporting requirements and provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov \302\267 The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations858@dss.ca.gov Assistant Director Jessica Beverly was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. On-line Licensing forms & regulations for a Child Care Center can be obtained on the Department\342\200\231s website: www.ccld.ca.gov . The Duty Officer is available to answer questions Monday \342\200\223 Friday; 8:00am to 5:00pm at: 1-844-LET-US-NO (1-844-538-8766) and/or 951-782-4200 If a civil penalty has been assessed during this inspection, payment is due when billed and the check(s) or money orders shall be made payable to the \342\200\234California Department of Social Services\342\200\235. You will receive an invoice in the mail. Do not send money until you receive your invoice. Do not send cash. The licensee/director is asked to update the following documents, if applicable, and submit to licensing within 30 days: 1. LIC 500 Personnel Report 2. LIC 610 Emergency & Disaster Plan 3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made or file copy is more than 2 years old) 4. LIC 309 Administrative Organization (only if changes have been made or file copy is more than 2 years old) 5. LIC 308 Designation of Administrative Responsibility (only if changes have been made & current designation is on file) See LIC809-D for cited deficiencies.
