Inspector’s narrative

What the inspector wrote

On August 21, 2025, at 8:30 a.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Annual inspection. LPA met with Facility Representative, Janette Maldonado and disclosed the purpose of inspection. LPA was led on a tour of the inside and outside the facility by Facility Representative. The facility operates in Classroom #1-#4, multipurpose room, and dining room. Room #1 had 6 children being supervised by one (1) Teacher and (1) aide. Room #2 had 3 children being supervised by one (1) Teacher. The facility is not operating in Classroom #3 or #4 due to low enrollment. Hours of operation are Monday \342\200\223 Friday 8:00 a.m. to 4:00 p.m. All toilets and hand washing facilities were observed to be in safe and sanitary operating condition. The floors in the facility were observed to clean and safe. Solid waste storage containers were observed to have tight-fitting covers and in good repair. The children bring their own lunch and the facility provides AM/PM snack. Drinking water is available both indoors and outdoors. Children bring their own water bottles. LPA observed areas around high climbing equipment does have sufficient cushioning material (artificial turf) surrounding the slide. Shade was observed in the outdoor activity space by the use of umbrellas and Pergola. The facility has a functioning carbon monoxide detector and a first aid kit that meet statutory requirements. The last fire drill was conducted and documented on 7/10/2025. Earthquake drill was conducted on 7/10/2025. Facility Representative stated that there are no swimming pools or other bodies of water on the premises. LPA did not observe any bodies of water. Facility Representative stated there are no firearms or ammunition allowed or stored on the premises. Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than twelve (12) children in care. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. The name of the facility representative and the fully qualified teacher designated to act in the facility representative\342\200\231s absence has been reported to the Department. LPA reviewed facility's physical sign in /sign out sheets/ procedures. Facility Representative was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children\342\200\231s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles. All facility staff do not have current documentation of completed mandated reporter training. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. A copy of lead testing verification was provided to LPA during the inspection. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility Representative states she will submit updated LIC 309 to the Department within 30 days. Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. One (1) Type B deficiency per California Code of Regulations, (Title 22, Division 12 & Chapter 1), is being cited on the attached LIC 809-D. A Notice of Site Visit (LIC 9213) was given to the Facility Representative, Janette Maldonado and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Facility Representative, Janette Maldonado.
