Incident investigation·January 30, 2026

On January 30,2026, at 1:02 p.m., Licensing Program Analyst (LPA) Angela Nguyen, conducted an unannounced case management inspection to follow up on an Unusual Incident that occurred on or about January 22,2026. LPA met with Director, Janette Maldonado and Licensee, Theresa Osborn. LPA disclosed the purpose of the inspection and toured the indoors and outdoors of the facility. The facility operates in Classroom #1- #4, multipurpose room, and dining room. There were no children present during the inspection due to a scheduled half day. Hours of operation are Monday \342\200\223 Friday 8:00 a.m. to 4:00 p.m. The incident that occurred on January 22, 2026 reported that part of the play ground fencing was destroyed by an elderly man in his vehicle. Director stated there were no children out on the playground or injured during the incident and a temporary chain linked fence was replaced and secured on the same day. Director stated the children were still able to use the outdoor play area the next day. Director stated that a new permanent fence is anticipated to be installed on February 4, 2026 and she will submit photos to the Department once the repairs are finalized. LPA inspected the outdoor play area and determined it to be safe for children to use and the temporary fencing was properly secured. No deficiencies cited during this inspection. A notice of site visit was given to the Director, Janette Maldonado. An exit interview was conducted and this report was reviewed with Director, Janette Maldonado and Licensee, Theresa Osborn.
