What the inspector wrote

On the above date and time, Licensing Program Analyst (LPA) Kelly Gerth arrived at the facility on a case management inspection, to follow-up on an Unusual Incident Report (UIR) which occurred on May 12, 2025. Upon arrival, LPA met with Site Director Janely Reyes and provided the purpose of inspection. The UIR stated that while playing in the outdoor activity space, staff observed a child who appeared to become lethargic. Staff was kneeled down to speak to the child when the child began to fall forward, staff then held the child and called out for support to other staff. Additional staff took over full supervision of remaining children in the activity space, while a 3 rd staff member called 911 and the family, as a 4 th staff member assisted directly with the child until emergency services arrived, with the family arriving shortly after. The child was transported by ambulance to the hospital for assessment and released the same day. LPA found that in addition to the appropriate measures taken by the CCC, the Site Director followed up with the family that evening and the following morning. The child was medically cleared and returned to care at the CCC within the same week. Based on the information obtained during the visit, there appears to be no violations of Title 22 Regulations pertaining to the reported incident and LPA determined that the facility took the necessary steps to ensure the safety of all children in care. An exit interview was conducted, a copy of this report, appeal rights and Notice of site visit was provided to Site Director Janely Reyes who was reminded it must remain posted for 30 days.
