What the inspector wrote

An exit interview was conducted, and this report was reviewed with Director Rodriquez. A copy of this report as well as a Notice of Site Visit (LIC9213) and Appeal Rights (LIC9058) were provided to the director and her signature on this form acknowledges receipt of these rights. LPA observed Notice of Site Visit being posted. Notice of Site visit must remain posted at the facility for 30 days.     Based on record review, the licensee did not comply with the section cited above in one out of three staff files reviewed do not contain current mandated reporter certificates. This poses a potential health, safety or personal rights risk to persons in care.

    101223 Personal Rights:(a) The licensee shall ensure that each child is accorded the following personal rights: (3)To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature...This requirement was not met as evidenced by: Based on interviews conducted by LPA, a staff member (S1) yelled or spoke to children with a raised voice on multiple occasions. This poses a potential health, safety or personal rights risk to children in care.

  • HEALTH-RELATED SERVICES

    Based on record review, the licensee did not comply with the section cited above in two out of three children with Epi-Pens on site do not have signed parent/guardian authorization forms. This poses a potential health, safety or personal rights risk to persons in care.
