What the inspector wrote

On 2/24/26, Licensing Program Analyst (LPA) Saraliz Velando conducted an unannounced inspection for the purpose of delivering findings for a complaint received on 12/10/25 regarding the above allegations. LPA met with the Asst Director, Rachel Fayak and toured the facility. There were 4 staff and 10 preschool children present. Appropriate ratios were observed. Staff members have the required background clearances and are associated with the facility. Based on the information obtained from staff interviews, there was corroborating evidence to substantiate all three allegations. There were staff that witnessed a staff member grab a child by the arm and pull him in an aggressive manner. There were also staff that witnessed at least two other children were yelled at while in care in an inappropriate manner and told inappropriate comments. Staff interviews also revealed there were multiple times when children were left unsupervised to either retrieve items from the front office or to have contact with the Director. The preponderance of the evidence has been met and therefore, the above allegations are found to be SUBSTANTIATED. Type B violations were cited. Refer to the next page LIC 809-D for deficiency citations. The exit interview was conducted with Asst. Director, Rachel Fayak. Appeal Rights and a copy of the licensing report were provided. A notice of site visit was posted and must remain for 30 days.

This poses a potential health, safety or personal rights risk to children in care.
