What the inspector wrote

Based on confidential interviews and records review the preponderance of evidence has been met and the allegation that staff inappropriately disciplined a child while in care is substantiated. The facility is being cited for title 22 section 101223 (a) (3) Personal Rights and 101173 (d) Plan of Operation Plan of Operation which poses an potential health and safety risk to children in care . This was not met as evidenced by record review and confidential interviews the children are being place in time out for at least a minute for each year of age when disciplined. An exit interview was conducted, this report, appeal rights was explained and handed to Director, Chandani Jayasinghe. The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post Type A reports for 30 days will result in a Civil Penalty of $100.00
