Incident investigation·February 21, 2025

On 02/21/2025 LPA conducted an unannounced visit for the purpose of following up on two Unusual Incidents. LPA met with Director, Alexa Denton and informed her of the reason for the visit. There were 31 children and 6 staff present during the inspection. Director accompanied LPA on a tour of the facility and playground. On 01/28/2025 C1 was left unattended on the preschool playground for approximately 1 minute. LPA interviewed Director and staff, took photographs of playground and collected facility records. This incident requires further investigation. On 02/11/2025 C2 a child received an injury to the pinky finger that required medical treatment. LPA interviewed Director, took photographs of play equipment, and collected facility records. This incident requires further investigation. No deficiencies cited. Exit interview conducted and copy of this report provided to Director. Notice of Site Visit must be posted for 30 days.
