What the inspector wrote

On 04/24/2025 LPA conducted an unannounced visit for the purpose of citing deficiencies noted in follow up investigation of an unusual incident. LPA met with Interim Director, Nichelle Gissler, and informed her of the reason for the visit. There were 31 children and 5 staff present during the inspection. On 01/29/2025, Director reported to the Licensing Duty Line that an unusual incident occurred on 01/28/2025 at approximately 10:55 am. Based on interviews and record review, Child 1 (C1) was left unattended on the secured preschool playground for approximately 1 minute as his classmates were being transitioned from the playground to the classroom. C1's absence was noted by Staff 1 (S1) upon arrival at the classroom and C1 was quickly retrieved and found to be unharmed. This incident was reported timely to Licensing and C1's parents. Deficiencies are being cited on the attached LIC 809D. Exit interview was conducted and copy of this report and appeal rights were provided to Interim Director. Notice of Site Visit must be posted for 30 days.

This requirement was not met as evidenced by, Based on interview and record review, the licensee did not comply with the section cited above when on 01/28/2025, C1 was left unsupervised on the playground for approximately 1 minute which posed a potential, safety, or personal rights risk to persons in care.
