What the inspector wrote

On 05/06/2026 at 2:00 PM, Licensing Program Analyst (LPA), Dana Stevens, conducted an unannounced Case Management Inspection to follow up on an Unusual Incident. LPA met with Interim Director, Reyna Ocampo, and informed her of the reason for the visit. Director accompanied LPA on a tour of facility and playground. There were 20 preschool children present with 3 staff during this inspection. On 04/29/2026, the Department received an Unusual Incident Report (LIC624) submitted by Director, reporting an incident that occurred on 04/28/2026. Director reported that on 04/28/2026 at 5:32 pm, on the Preschool playground, Child 1 (C1) exited the preschool playground to the parking lot through a gate that was left open as Parent 1 (P1) entered the playground. Director stated C1 was immediately retrieved by Parent 2 (P2), who was present on the playground to pick up their child. During the Unusual Incident review, LPA toured facility and playground, conducted interviews with the Director and staff, and collected facility records. This incident requires further investigation. Director will email current facility roster (LIC 9040). No deficiencies cited during today's inspection. Exit interview conducted and copy of report and appeal rights provided. Notice of Site Visit must be posted for thirty days.
