What the inspector wrote

On 10/30/2025, Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced case management inspection for the purpose of citing deficiencies. LPA met with Director, Alexa Denton. Director accompanied LPA on a tour of the inside and outside of facility. There were 20 children present with 6 staff members. During inspection LPA observed there were 10 infants present in Infant Room 1 (0-12 months) and only 5 cribs were available. LPA observed sleep mats were also available. During interviews Director and staff stated 2 infants in Room 1 are near 12 months old and sleep on mats, and the remaining 8 infants sleep in cribs. Deficiencies are being cited on the attached LIC 809-D. Exit Interview conducted and copy of this report and appeal rights provided to Director. Notice of site visit must be posted for 30 days.

This requirement was not met as evidenced by, Faciity did not provide enough cribs to meet the requirement of a crib for every infant that is unable to climb out of a crib, which poses a potential health, safety or personal rights risk to children in care.
