What the inspector wrote

Regarding the allegation that staff did not provide a safe and comfortable environment for children, based on interviews conducted it was disclosed that there are always 2 staff members with the children at all times to ensure the needs of the children are being met. Lastly, regarding the allegation that the facility is in disrepair, based on observation it appears that duck tape is used to cover parts of the Exit sign and baseboard. Interviews conducted disclosed that duck tape is covering parts of the Exit sign to avoid children from picking at it, and duck tape was placed on the base board to keep it from peeling off. LPA did not observe any disrepair in the facility. Based on the information obtained during this investigation, it has been determined that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur. Therefore, the allegations are UNSUBSTANTIATED . An exit interview was conducted, and this report was reviewed with the Authorized Representative, Vipula Rajapakse , and a copy was provided. Appeal rights were discussed and provided during the exit interview. A Notice of Site visit was given, and Authorized Representative understands that it must remain posted for 30 days.
