What the inspector wrote

On 1/26/26 at 9:30 am Licensing Program Analyst (LPA) Gerald Poindexter made an unannounced case management visit, while at the facility on another matter. LPA met with Giavanna Aguilar, assistant director. Elizabeth Smith, center director, was not present today, but was contacted about the visit by phone. There were no children present, as the program start time is at 3 pm. The purpose of the visit was to follow up on a 1/16/26 report to the Department of an unusual incident that occurred on 7/25/25. Specifically, the report detailed an incident at the facility in which a parent was potentially verbally and physically abusive toward their child in the presence of staff and other children. Ms. Aguilar states that she was aware of the incident, but acknowledged only having been present for a portion of the incident and not the full extent. Further information is required. Ms. Aguilar referred LPA to Ms. Smith for additional information about the incident. No deficiencies cited. Exit interview conducted and report was reviewed with Giavanna Aguilar, center director. A Notice of Site Visit was given and must be posted for 30 days. Appeal Rights provided.
