What the inspector wrote

On 07/16/2025 at 1:50 pm, Licensing Program Analyst (LPA) Adriana Macias, conducted an unannounced case management inspection to follow up on a self-reported unusual incident. Incident took place on 6/11/2025 and was self reported on 6/12/2025. Upon arrival, LPA met with Director Elizabeth Smith and Assistant Director Giovanna Aguilar and discussed purpose of visit and toured the facility. During inspection, there was a total of 20 children and 3 staff. LPA found facility to be within appropriate ratios and staff to be fingerprinted and associated. Incident reported involved a child (C1) who was left unattended in the director’s office. Based on parent and director's interview, C1 was left unattended for less than a minute inside the director's office because director had stepped out of the building to attend to a non-related manner. During today’s inspection, LPA observed the director's office next to a classroom, where both rooms are divided by half a wall and a door. Per director, the door that connects both rooms had been left ajar during the incident and there was a teacher supervising a couple of children in the classroom that connects. The director stated that she came back inside the room right away and found C1 contempt and ready to start the day. Director stated that C1 had not been having a great start the day of the incident, therefore during drop off, child had been allowed to stay seated inside the office with the director for a little bit, while C1 felt better and was ready go inside the classroom. Based on LPA’s inspection, parent and director interviews, a Technical Violation was issued. (See LIC9102TV for technical violation) Exit interview conducted and report was reviewed with Director . LPA provided a copy of the report and appeal rights. A notice of site visit was provided and must remain posted for 30 days.
