Based on the information obtained from file review and interviews with relevant parties, the above allegation could not be verified and is found to be UNSUBSTANTIATED . Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Exit interview conducted and report was reviewed with Giavanna Aguilar, assistant director. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights and copy of this licensing report were provided.
