What the inspector wrote

On April 2, 2026, at 12:00 PM, Licensing Program Analyst (LPA) Jennifer Irving conducted an unannounced case management inspection to follow up on an unusual incident that was self-reported to the Department on March 26, 2026. The purpose of the visit was disclosed. Entry was granted by Director Elizabeth Smith and assistant director, Gia Aguilar. LPA toured the facility, conducted interviews, and reviewed relevant documentation. A census was taken. There were 29 preschool-age children present, supervised as follows: Ladybugs Classroom (2-year-olds): 10 children with 3 staff Frogs Classroom (3-year-olds): 13 children with 3 staff Owls Classroom (4-year-olds): 6 children with 1 staff The facility was observed to be operating within licensed capacity and in compliance with required staff-to-child ratios. Children were observed to be under appropriate care and visual supervision. Per interview with Director Elizabeth Smith, on March 25, 2026, during nap time, staff (S1) reported that child (C1) informed staff that they had placed a pinto bean, picked up from the classroom floor, into their ear. Staff immediately notified the Director. The child’s parent(s) and the Department were notified, and an unusual incident report was submitted. Director stated the child was taken to receive medical evaluation, where the pinto bean was removed. No complications were reported. Staff stated that visual supervision was being maintained at the time; however, the incident was not directly observed. The information was reported by the child to staff. Based on observations, interviews, and record review conducted during today’s inspection, no deficiencies were cited. An exit interview was conducted with Director Elizabeth Smith and Gia Aguilar. A copy of this report and appeal rights were provided. A Notice of Site Visit was issued and shall remain posted for 30 days.
