What the inspector wrote

On April 20, 2026, at 12:52 p.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Annual- Required inspection. LPA met with Facility Representative, Janessa Richardson and disclosed the purpose of inspection. LPA was led on a tour of the inside and outside of the facility by the Facility Representative. There were two napping infants were being supervised by one (1) teacher in Classroom #4. Classroom #3 is not currently in use due to low enrollment. The facility serves children ages 6 weeks – 24 months. Hours of operation are Monday – Friday 7:00 a.m. to 6:00 p.m. Disinfectants, cleaning solutions, and other hazardous items were made inaccessible to the children in care. No poisons were observed during the inspection. Furniture and equipment in the classroom were observed to be in good condition, free of sharp, loose or pointed parts. Infant changing table were within arm’s reach of the sink. LPA observed an age appropriate play structures on the playground. Areas around high climbing equipment were observed to have sufficient cushioning material by the use of rubber cushioning to absorb falls. Shade was observed in the outdoor activity space by the use of a canopy over the entire play area. The facility provides breakfast, lunch and snack. Menus are posted weekly. Solid waste containers were observed to have tight-fitting covers and in good repair. Drinking water is available both indoors and outdoors. Facility Representative stated that there are no swimming pools or other bodies of water on the premises. Facility Representative stated that there are no firearms or ammunition allowed or stored on the premises. The facility has a functioning carbon monoxide detector and a first aid kit that meet statutory requirements. The last fire drill was conducted and documented on 03/03/2026. Last earthquake drill was conducted and documented on 03/04/2026. For additional information regarding the inspection and its tools and methods, please visit the Program website at www.cdss.ca.gov/inforesources/community-care-licensing/insp Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than four (4) children in care. The name of the facility representative and the fully qualified teacher designated to act in the facility representative’s absence has been reported to the Department. LPA reviewed facility electronic sign in /sign out sheets. Facility Representative was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children’s files and observed the files were complete with contact information for authorized representative and/or relatives or others who can assume responsibility for medical assessment. Bottles containing formula contain the child’s name and date. Infant Needs and Service Plans were current and up to date. Each crib or cot is occupied by only one infant at a time and cribs are kept free from all loose articles including blankets and pillows and there are no objects hanging above or attached to the crib. Infants are not swaddled while in care. Staff physically checks on sleeping infants every fifteen minutes and documents any signs of distress which includes but is not limited to flushed skin color, increase in body temperature, restlessness and labored breathing. Documentation for infants up to 12 months includes sleeping position if it is other than on their back. Individual Infant Sleeping Plan is completed and in the file for each infant up to 12 months of age. Infants up to 12 months of age are placed on their backs for sleeping. Staff-infant ratio requirements are being met while infants are sleeping. LPA observed 15-minute sleep checks were documented. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP.
