What the inspector wrote

On April 20, 2026 at 10:00 AM, Licensing Program Analyst (LPA) Angela Nguyen conducted an unannounced Case Management inspection to follow-up on a self reported incident that occurred on or about April 7, 2026. LPA disclosed the purpose of the inspection and was granted a tour into the facility by Facility Representative, Janessa Richardson. There were 2 children being supervised by 2 staff members. On April 8, 2026, Facility Representative reported that a staff member inappropriately mishandled a child in care by pulling them by the hair with aggression and pulling them by the arm to remove from her presence. Facility Representative stated that the staff member was immediately removed and was terminated and the child's parents were notified on the same day of the incident. Interviews were conducted with the Facility Representative and staff members. Based on the video footage captured and reviewed and corroborating statements, a personal rights violation did occur. Type A deficiency was issued during today's inspection, see LIC809D. LPA Angela Nguyen informed facility representative, Janessa Richardson that this report dated April 20, 2026 that documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Angela Nguyen informed the facility representative to provide a copy of this licensing report dated April 20, 2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Notice of site visit (LIC9213) was given and must remain posted for 30 days. Exit interview was conducted and this report was reviewed with Facility Representative, Janessa Richardson.
