What the inspector wrote

On 05/13/2022 at 12:15 PM, LPA Dana Stevens conducted an unannounced complaint inspection to deliver findings on the above allegation. LPA met with Director Stephanie Freeman. There were 20 children in care at the time of the inspection. During the course of the investigation, interviews were conducted with the reporting party, daycare children, daycare parents, daycare teachers, and Director. LPA reviewed facility records and documentation provided by Reporting Party. Based on information obtained in interviews and review of facility records the facility is co-mingling school-age children with pre-school children routinely in the last 30-45 minutes of the day, thus this allegation is found to be Substantiated. Because an incident occurred during the co-mingling time period in which a preschool child was injured a Type A citation is being cited on the attached LIC9099D. Upon Receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. An exit interview was conducted and copy of this report was provided to Director. Notice of Site Visit must be posted for 30 days.

    101161(a) Limitations on Capacity and Ambulatory Status. The licensee shall not exceed the conditions, limitations and capacity specified in the license. This requirement was not met as evidenced by: Facility's capacity conditions were violated when it was determined preschool and school-age children were commingled routinely in the last 30-45 minutes of the day. This poses a immediate risk to the children in care.

Events reported shall include...Any injury to any child that requires medical treatment.This requirement was not met as evidenced by, Licensee did not submit a verbal or written report of the incident to Licensing department.
