What the inspector wrote

The first allegation is that Staff are not following infant needs and services plan. Interviews revealed that the needs and service plan are updated every three months or earlier if needed. Further interviews revealed that any new special requests/orders are included in the plan immediately and the necessary steps are taken to ensure it is followed. A review of children records was conducted, and LPA verified that the needs and service plans were updated as of date of inspection. In regard to C1 being left on the carpet without a blanket, interviews revealed that there was mis-communication between the parent and staff. The staff were unaware of the doctor\342\200\231s note because the parent did not disclose the information to the director. According to staff, once the doctor\342\200\231s note information was provided, staff were made aware and followed the doctor\342\200\231s instructions. The second allegation is Staff are not maintaining facility carpets clean. Facility tour, pictures, and interviews revealed that necessary steps are taken to ensure the carpets in the infant care room are maintained and cleaned regularly. Interviews revealed that the carpets are vacuumed daily after 5 PM and shampooed every two months. Interviews also revealed that the carpets are shampooed earlier if there is a need. On 02/22/24, LPA conducted a tour of the facility, and took pictures which did not reveal any stains or dirt marks on the rugs in the infant classroom. From the information received through interviews with staff, and facility documents the above allegations cannot be verified. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the allegations did or did not occur, therefore, the allegations are UNSUBSTANTIATED. An exit interview was conducted with Facility Director Angela Hunt, a Notice of Site Visit posted, and a copy of this report was provided to the facility on this date and time.
